  • Uniform application for Agency Link to external website or Online application
  • Registration with the Idaho Secretary of State (see SOS Reference Sheet for more information)
  • At least one registered licensed producer (DRLP) must be listed on the application
  • Fee: $80. If applying by paper, checks should be made payable to the Idaho Department of Insurance and application and attachments should be mailed to:
