Branch Office Notification: Use this form to report additional locations that will be operating under the same FEIN/Name. It is optional to report Branch Offices. Although the form asks for a DRLP for that location, it is still your responsibility to register them to your agency.
Change of Officer Notification: Use this form to update the officers and directors of your agency. TPA’s must include bio affidavits for new officers.

DBA Add Request: If you’d like to have a “Doing Business As” name listed on your license, you must complete this form. Please note we will not process this request without a completed Idaho Secretary of State filing. Please attach the completed form. Non-residents must also provide proof of filing from their domiciled state.

Line of Authority Cancellation: Use this form to remove a line of authority from your license.
Name Change: This form may only be used when there is no change in FEIN or entity structure of the business. If the structure or FEIN number has changed, please contact our department for further directions on how to proceed. This form requires you to provide a copy of the amended Secretary of State Filing from both Idaho and your domiciled state (for non-residents) as well as a list of current officers.
Producer Appointment: This form allows you to appoint a producer (either an agency or an individual) to a carrier. These can also be done online through Sircon.
Producer Registration: This form allows you to register a producer to work under your agency license. These can also be done online through Sircon.
Producer Termination: This form terminates either the producer’s registration to work under an agency license or the producer or agency’s appointment to a company.
Voluntary Surrender of Business License: Use this form to cancel your Idaho Business Entity license. Please note, there is no penalty in Idaho for non-renewal of a license. It is usually recommended to simply let the license expire rather than complete a voluntary surrender request.
